New Clients: I ask that you arrive 15 minutes before your scheduled appointment time to allow for intake process, including filling out a new client form.
**If you have access to a printer I have provided a link below to the client intake form for you to fill out prior to your appointment.
Existing Clients: I ask that you arrive 5 minutes before your scheduled appointment time to allow for intake.
Reschedule or Cancellations
I kindly ask that you contact me 24 hours in advance to cancel or reschedule an appointment. If you are running late please contact me. If you are 15 minutes late you will be considered a no show and full payment will be due.
